What is Support Coordination?
Support Coordination is all about enhancing your ability to manage and direct your own NDIS services.
Our service will assist you to:
Find providers, understand the flexibility within each budget category, work through any challenges you may be experiencing, coordinate a range of funded, community and mainstream supports and help you to realise the full potential of your NDIS plan.

What is Specialist Support Coordination?
Specialist Support Coordination is a unique form of assistance offered through the National Disability Insurance Scheme (NDIS). It's designed for participants facing more complex situations in their lives, requiring additional support.
Similar to regular Support Coordination, it assists participants in identifying appropriate services and support. However, there are distinct features that set it apart.

What is Younger Onset Dementia (YOD)?
Younger Onset Dementia (sometimes also called Early Onset Dementia) is the term used to describe any form of dementia diagnosed in someone under the age of 65.
Due to the highly complex nature of YOD, we believe an in depth understanding of its unique presentations, as well as an in depth knowledge of how to best work within the framework of the National Disability Insurance Scheme, is required to best support those living with the condition, as well as their families, friends and loved ones.

Led by an experienced Registered Nurse, our General and Specialised Support Coordination services
extend across various regions, including Macedon Ranges & Surrounds, Sunbury & Surrounds,
Melton & Surrounds, Bendigo, Castlemaine & Surrounds, and Metropolitan Melbourne Areas.
Our Y.O.D Support Coordinators offer to service to all Victorians.
